1. Scope of Services

• Psychiatric Care: We provide psychiatric evaluations, medication management, and supportive therapy for teens and adults.

• Not an Emergency Service: Faithful Wellness Solutions is not an emergency psychiatric facility. If you are experiencing a life-threatening mental health crisis, please call 988, 911, or go to the nearest emergency room.

• Telehealth: Services provided via video are subject to the same clinical standards as in-person visits and require a secure, private connection.

2. Patient Responsibilities

• Accuracy of Information: You agree to provide accurate medical history and contact information to ensure safe and effective treatment.

• Conduct: We maintain a professional, respectful environment and expect the same from our clients.

• Portal Use: You are responsible for maintaining the confidentiality of your login credentials for our secure patient portal.

3. Financial Terms

• Self-Pay Rates: Fees for initial evaluations ($300) and follow-up visits ($150) are due at the time of service unless otherwise arranged.

• Insurance: We accept Aetna, Cigna, UnitedHealthcare, Oscar, and Oxford via Headway. It is your responsibility to verify coverage prior to your appointment.

• Cancellation Policy: Please provide at least 24-hour notice for cancellations to avoid potential late-cancellation fees.

4. Clinical Partnership

• Collaboration: Treatment is a partnership; we walk alongside you to problem-solve and navigate your healing journey.

• Medication Compliance: Patients agree to follow prescribed medication protocols and attend scheduled follow-up visits for safety and monitoring.

5. Limitation of Liability

• Faithful Wellness Solutions and Katrina Kilburn, PMHNP-BC, are not liable for technical issues arising from your internet service during telehealth sessions.

• We do not guarantee specific outcomes, as mental health healing is an individualized process.

6. Changes to Terms

• We reserve the right to update these terms as necessary. Continued use of our services constitutes acceptance of the revised terms.
